Requesting Your Health Care Records
You have the right to access your mental health records. To request copies of your records, submit a written request via email to bbh@branbehavioralhealth.com, or by mail to 9711 Mason Dr. #125 Box 367, Richmond, TX 77407. Please include your full name, date of birth, and specify the records you are requesting. Requests are typically processed within 15 business days, and a reasonable administrative fee may apply for copying and processing. If you have questions about your records or need assistance with your request, please contact us.
